Job Description

The Redding Rancheria is seeking a Community Health Worker. The preferred schedule for this position is Wednesday 1:00 PM - 5:00 PM and Thursday and Friday 8:00 AM - 5:00 PM.

The listed wage reflects the entry-level rate for this position. Compensation may be adjusted based on experience, education, and qualifications.

JOB SUMMARY

The Community Health Worker will be responsible for acting as a bridge between healthcare providers and the community to facilitate access to services and improve the quality and cultural competence of service delivery. This role will focus on community outreach, preventive health education, basic diagnostic procedures, and patient monitoring.

REQUIREMENTS/MINIMUM QUALIFICATIONS

  • High school diploma or equivalent required; some college or vocational training preferred.
  • One year of experience working in a community-based setting preferred.
  • Certified as a Community Health Worker or willingness to complete certification within the first year of employment.
  • Strong interpersonal, communication, and organizational skills.
  • Basic understanding of healthcare systems and medical terminology
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid driver’s license and reliable transportation
  • Must pass pre-employment drug screening.
  • Indian preference.

ESSENTIAL JOB DUTIES

  • Assist in identifying and helping to address social determinants of health affecting community well-being.
  • Provide basic healthcare services such as blood pressure monitoring, health screenings, and vaccinations.
  • Collaborate with other healthcare professionals to ensure a comprehensive approach to patient care.
  • Offer referrals and coordinate with social services, housing services, and other community internal and external resources.
  • Maintain accurate and up-to-date records of patient interactions.
  • Organize and lead community health workshops and educational sessions.
  • Ensure strict confidentiality of all patient information.
  • Receive all program visitors and patients, offering information and/or answering questions as needed.
  • Assist with coordination and implementation of all Community Health events and classes.
  • Coordinate services with Community Health representatives, mental health and other social services agencies to render effective continuity of patient care.
